The team researches and vets available dietitians and evaluates them for quality, availability, and cost. Once they have a shortlist, the team interviews several dietitians and facilitates virtual meetings so Benny can make an informed choice, ultimately advising him on one that fits his needs.

Navigating a chronic condition group class

Benny joins an expert-led peer support group for older adults with diabetes. In this group he learns about wound care, healthy eating, and ways to stay active.

In this class led by Cleo’s Chronic Conditions Specialists, Benny:

  • Learns how to manage and navigate his diagnosis
  • Gets information about treatment options, including the importance of asking questions and seeking second opinions
  • Explores symptom management and comfort strategies
  • Gets tips for communication and emotional support
  • Learns how to find joy amidst challenges

Benny leaves the group with strategies for coping with change and maintaining a sense of normalcy with diabetes.

Improved emotional wellness and connection

Cleo knows that social isolation is a major issue for the aging population. Cleo administers a regular PHQ-4 to monitor Benny’s emotional wellness and helps him find a local bird watching group to stay connected to his community and explore one of his passions.

In-home care

With no family close by or local support, Benny struggles to get things done around the house. Cleo alleviates this strain by:
