George's story

George is 76 years old and lives in a rural community in New Mexico. George is struggling to maintain a healthy diet and lifestyle on a fixed income.

His work as a construction foreman has led to musculoskeletal challenges, and he also faces issues with balance. Understandably, his family worries for his safety.

Since his wife passed away, George does not get out of the house or socialize often.

George’s son, Jason, lives in the Northeast and is trying to support his father from a distance.

Getting set up

George receives an email from Cleo and taps to enroll through his device. He selects support for fall risk and nutrition. After being prompted to add anyone in his family who is helping him, George adds his son, Jason, as a caregiver.

Assessing George's needs

George and Jason receive a group message in the app with a welcome from a Cleo Guide. They set up a call to talk through George’s needs. On the call, George’s Cleo Guide conducts a needs assessment including a PHQ-4, which shows that George scores positive for being at risk for depression. Based on these results and other information, the Cleo app is updated to include information and resources specific to George’s needs.

Getting George the help he needs

George’s Guide identifies that he needs help with his emotional wellness, managing his A1C, getting a home safety assessment, and securing in-home care.

Based on these needs, the Guide works with Cleo’s Care Activity Team and secures the following for George:

  • An in-network therapist
  • A spot in Cleo’s health and wellbeing coaching group
  • A nutrition session with a Cleo Specialist
  • An in-home safety assessment
  • An in-home health aide

How does Cleo’s Care Activity Team work?

George’s Guide works with the Care Activity Team to find and schedule the support he needs. In addition to booking an in-home safety assessment for George, they also vet and research home health aides based on the criteria that George needs, and help him in making a final decision. The team also finds and evaluates a therapist for George based on cost and approach. At every turn, the team makes sure George feels empowered and supported.

Continued care for George

Cleo’s group sessions are designed to both combat isolation and provide actionable steps for members to improve their wellbeing.

George signs up for a weekly Cleo Connect where a Cleo Specialist facilitates conversations and learning. Through this weekly group George gets:

  • Connection time with others older adults
  • New nutritious recipes to try
  • Help putting together a list of questions for his healthcare team
  • Skills to live a healthy lifestyle

Peace of mind for George’s family

Jason has biweekly calls with George’s Cleo Guide to get updates on his dad’s progress. During these calls, Jason can also request support or express new concerns. Cleo’s approach to whole family health means relieving the caregiving burden for our members’ families so they can enjoy their time with their loved ones.

Connected care

Cleo integrates seamlessly into George’s network of care. After connecting with George’s Medicare case manager, the Guide gets updated care plans from his healthcare provider and fulfills referrals by finding in-network available providers. His Guide also finds local resources like home health aides and meal delivery services to ensure the most cost effective support.

Safer home

  • Cleo’s Care Activity Team found and scheduled with a local nonprofit to secure the loose rugs, install a railing, and replaced the tub with a walk-in shower in George’s home.

Lowered A1C

  • The skills George learned in the 6-week health and wellbeing group coaching class helped him take control of his diet, leading to a lower A1C at his annual physical.

Improved emotional wellness

  • After 6 months of Cleo’s support, George’s PHQ-4 score improves thanks to bi-weekly therapy as well as the Cleo Connect support group George attends weekly. George also reports feeling less like a burden to his family because he knows he can turn to Cleo for help.
