Grace is a university professor with two kids aged 4 and 12. Her 12-year-old was recently diagnosed with ADHD.

She and her husband are also starting to care for her mother-in-law who is showing signs of dementia and has had a recent fall.

Cleo’s Family Health Index drives individualized support for Grace

The Family Health Index is Cleo’s proprietary quantitative caregiver health assessment. It measures family health across seven dimensions, allowing Cleo to analyze and be proactive about the specific areas in which members need support. The FHI defines Grace’s baseline and allows her Cleo team to see a snapshot of her overall wellbeing.

For Grace, her FHI results show that she has needs around Self Care, Caregiving Balance, Connectedness, and General Health. Both the app and live support are customized to Grace’s needs through Support Plans and sessions.

Led by Cleo Coaches, this six-week group education series covers:

  • Identifying what matters most to you
  • Creating your health and wellbeing vision
  • Setting actionable goals
  • Experiencing sustainable and lasting change
  • Checking in on your goals and adjusting

Referral to other benefits

Cleo focuses on coaching through and to care, actively referring members to in-network benefits. Through the benefits hub in the Cleo app, Grace gets referred to her EAP for mental health support. Her Guide coaches her on how to get the most out of this experience and finds other in-network benefits such as free mental health resources.

Identifying needs to personalize care

Assessments help Cleo meet the needs of our members. Assessments are designed to be proactive by identifying issues members may not even know they need to address, as well as reactive by allowing members to identify what is most important to them.

For Grace, she has questions about early intervention, school support, and navigating the evaluation and diagnosis process. This triggers Grace’s Guide to take action.

Navigating an IEP

Grace needs help with an upcoming IEP meeting

Grace’s Guide sets up call with Neurodivergence Specialist. The Cleo ND Specialist talks to Grace to understand her concerns and questions. The Specialist provides education on what to expect at an IEP meeting and gives Grace tips and personalized, in-app resources. The Specialist also helps Grace create a list of questions and goals ahead of the IEP meeting.

Working with the Care Activity Team, the Specialist finds an ND-affirming, strengths-based tutor for PSAT prep for Grace’s teen. After the IEP meeting, the Specialist works with Grace to review and go over next steps so Grace feels prepared and empowered.

Thanks to Cleo, Grace reports feeling more balanced and better able to show up at work, lowering her risk of burnout. Her FHI score reflects this with a 12% overall increase, with marked improvement in Caregiving Balance and Connectedness.
